Below is our library card application. Fill it out to apply for a "virtual card." Once you have applied, please do not apply again. We will always keep your library card on file in our Sacramento office and we will reference it when you call us to check out materials.

Please read the following statement and, if you understand and agree, check the "I agree" box below:

I understand that the Adoption Resource Library is made possible through a grant from the Sacramento County Department of Health and Human Services. The library is available to all birth parents, adoptees and adoptive parents residing in Sacramento County and to families who are living outside of Sacramento but have adopted a dependent child from Sacramento County. Mental health professionals may also use the library. Library books may be checked out for three weeks and videos for two weeks. It is my responsibility to return the materials on time and in good condition. Failure to do so may result in loss of library privileges. If an item is lost or damaged, I will be billed and I agree to pay the replacement cost of the item, plus shipping and handling charges.
